③details of any defects observed before the tests;测试中遇到的问题点

④details of any defects and damage observed after the tests;整改建议

⑤the test result;测试结论

⑥details of any deviation from the test proceduresg测试不合格的细节

⑦name and address of test facility申请方抬头地址

⑧date of testing测试日期
